Use this form to submit situations that require review by the Captains Council. Examples include rule disputes, sportsmanship concerns, disciplinary matters, eligibility questions, and other league-related issues.

Please provide as much information as possible. Submitting a request does not guarantee action will be taken. The Captains Council will review the information provided and determine if further investigation or action is warranted.

 Please note: Not every disagreement requires Captains Council review.

Most issues can and should be resolved between players, captains, and officials on game night. This form should be reserved for matters that materially affect league operations, player safety, sportsmanship, or competitive integrity.
